依那西普减量的新标志

本研究的目的是探讨骶髂关节（SIJ）骨髓水肿（BME）与临床特征和临床反应的相关性，以及骨髓水肿的快速减少是否可作为强直性脊柱炎（AS）患者依那西普剂量逐渐减少的新标志。

90例接受6个月依那西普治疗的活动性强直性脊柱炎患者连续入组，分为标准剂量组（n=37）和剂量逐渐减少组（n=53）。骶髂关节骨髓水肿和临床反应分别通过加拿大脊柱骨关节研究协会评分系统(SPARCC)和国际脊柱关节炎学会评估改善40%(ASAS40)进行评估。“骶髂关节骨髓水肿的快速下降”定义为SPARCC评分≥M0降至M1的50％。

骶髂关节骨髓水肿与VAS疼痛评分，BASDAI评分，CRP，IL-1β，IL-17和TNF-α水平呈正相关。剂量逐渐减少组中M6的ASAS40应答率低于标准剂量组，而骶髂关节骨髓水肿快速减少的患者高于其他患者。此外，剂量渐减组的ASAS40应答率与骶髂关节骨髓水肿快速降低的患者的标准剂量组相似，但低于M6的骶髂关节骨髓水肿没有快速降低的患者的标准剂量组。

骶髂关节骨髓水肿的快速下降预示着对依那西普的治疗反应较好，可作为强直性脊柱炎患者开始依那西普减量的新标志。

Objective

Thepurposeofthisstudywastoinvestigatethecorrelationofbonemarrowedema(BME)insacroiliacjoint(SIJ)withclinicalcharacteristicsandclinicalresponse,andwhetherthequickdecreaseofBMEcouldbeservedasanovelmarkerfordosetaperingofetanerceptinankylosingspondylitis(AS)patients."

Methods

NinetyactiveASpatientsunderwentetanercepttreatmentfor6monthswereenrolledconsecutivelyandclassifiedintostandarddosegroup(n=37)anddosetaperinggroup(n=53).BMEinSIJandclinicalresponsewereassessedbySPARCCcriteriaandASAS40responsecriteria,respectively.“QuickdecreaseofBMEinSIJ”wasdefinedasthedecreaseofSPARCCscore≥50%fromM0toM1."

Results

BMEinSIJwaspositivelycorrelatedwithpainVASscore,BASDAIscore,CRP,IL-1β,IL-17,andTNF-αlevels.ASAS40responserateatM6waslowerindosetaperinggroupthanstandarddosegroup,whilehigherinpatientswithaquickdecreaseofBMEinSIJthanotherpatients.Besides,theASAS40responserateindosetaperinggroupwassimilartostandarddosegroupinpatientswithaquickdecreaseofBMEinSIJbutwaslowerthanstandarddosegroupinpatientswithoutaquickdecreaseofBMEinSIJatM6.

Conclusion

AquickdecreaseofBMEinSIJpredictsbettertreatmentresponsetoetanercept,anditmightbeservedasanovelmarkerfordosetaperinginitiationofetanerceptinASpatients.
